Description

  • Serve as the primary point of contact for a panel of clients throughout their care journey.
  • Guide clients and families through onboarding and ongoing service needs.
  • Maintain regular contact to manage scheduling, adapt care plans, and support a positive client experience.
  • Build strong relationships with Care Professionals to ensure quality matches and consistent staffing.
  • Partner daily with frontline support and staffing teams to maintain reliable schedules and timely updates.
  • Manage escalations and complex care situations with professionalism, sensitivity, and clear communication.
  • Work cross-functionally with internal teams to improve processes and share feedback on operations, product, and HR/admin workflows.
  • Build and maintain relationships with partner agencies and translate client needs into actionable care plans.
  • Provide real-time updates and regular check-ins on client status, issues, and care plan changes.
  • Triage requests, set expectations, and help resolve tickets, billing issues, and follow-up tasks.

Requirements

  • 3+ years of experience in senior care, patient management, or customer service.
  • Client-first mindset with a strong commitment to quality care and customer satisfaction.
  • Strong ownership mindset and detail orientation with experience managing complex processes.
  • Confidence in handling escalations, solving problems creatively, and building trust with clients and partners.
  • Excellent organizational skills and ability to manage multiple priorities in a fast-paced environment.
  • Ability to make sound decisions under pressure.
  • Comfort with technology; experience using scheduling, CRM, or workforce management systems is a plus.
  • Flexibility to support clients and partners during evenings or weekends if needed, including support on two holidays per year.

  • Remote position with a Monday–Friday, 8:00 a.m. to 5:00 p.m. Eastern Time schedule.
  • Hiring salary range of $64,000–$69,000 USD.
