Description

  • Serve as the primary point of contact for enrolled patients and build trust through consistent outreach.
  • Conduct regular check-ins to assess health status, care needs, and social barriers.
  • Manage a caseload of approximately 75-150 patients and prioritize outreach based on acuity and care gaps.
  • Coordinate appointments, provider communication, referrals, prior authorizations, and medication changes.
  • Support medication adherence and help patients access affordable medications and durable medical equipment.
  • Connect patients with transportation, meal programs, community resources, benefits programs, and social services.
  • Conduct structured social determinants of health screenings and follow through until needs are resolved.
  • Provide condition-specific education and self-management coaching tailored to each patient.
  • Document all patient interactions, interventions, barriers, and outcomes accurately and in real time.
  • Work across multiple systems, troubleshoot basic technology issues, and suggest workflow improvements.

Requirements

  • Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential from a nationally recognized certifying organization.
  • 1+ years of experience in remote care coordination, Chronic Care Management (CCM), Remote Patient Monitoring (RPM), care navigation, or a similar patient-facing role.
  • Experience working with Medicare-enrolled or dual-eligible populations.
  • Ability to build trust and communicate effectively with older adults and people managing serious chronic conditions.
  • Strong understanding of care coordination principles and healthcare workflows.
  • Comfort discussing chronic conditions, medications, and treatment plans without providing medical advice.
  • Proficiency with EHR systems, care management platforms, CRMs, and digital communication tools.
  • Self-directed, metric-aware, and able to manage caseloads independently.
  • Comfort with ambiguity, rapid iteration, and evolving processes.
  • Reliable internet and a professional home workspace for remote work.
  • High school diploma or equivalent required; associate's or bachelor's degree in healthcare, social work, public health, or related field strongly preferred.
  • Must be located in FL, GA, NC, TN, or TX.
  • Community Health Worker (CHW) certification or training preferred.
  • Experience with post-hospital or post-ED transitional care calls preferred.
  • Experience with SDOH screening tools or community resource navigation preferred.
  • Bilingual Spanish strongly preferred; other languages may be valuable depending on the population.
