Description

  • Provide telephone triage support for hospice patients and families after hours.
  • Assess patient conditions and determine appropriate next steps for care.
  • Coordinate with on-call teams to arrange interventions, resources, and follow-up.
  • Accurately document calls, communications, and nursing interventions in the EMR.
  • Maintain a calm, professional, and empathetic demeanor during urgent calls.
  • Support timely patient care coordination for remote hospice and home health clients.
  • Work scheduled overnight and weekend shifts as part of the triage rotation.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience is strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to manage stress and multitask while handling high call volume, including at least 5 calls per hour on weekdays and up to 8 per hour on weekends.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity required for all scheduled shifts.

Benefits

  • Base pay of $25 per hour with opportunities to increase to up to $28 per hour within the first 6 months.
  • $2 shift differential for overnight shifts.
  • $2 shift differential for weekend shifts (Friday evening, Saturday, and Sunday).
  • Paid remote training for 3 weeks.
  • PTO accrual for both part-time and full-time nurses.
  • 401(k) plan eligibility for part-time and full-time nurses.
