Description

  • Provide telephone triage support for hospice patients and their families after hours.
  • Assess patient conditions and determine appropriate next steps based on calls received.
  • Coordinate care and resources with on-call teams to support patient needs.
  • Document all calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, empathetic, and professional demeanor during urgent or stressful calls.
  • Work independently while supporting timely patient care coordination in a remote setting.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience is strongly preferred.
  • Comfortable using technology and electronic medical records (EMR) for call documentation.
  • Able to type documentation and notes in a fast-paced environment.
  • Able to handle stress and multitask while receiving a high volume of calls.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity required for all scheduled shifts.
  • Must be able to manage at least 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

Benefits

  • Base pay of $25 per hour once trained, with potential to earn up to $28 per hour within the first 6 months.
  • $2 shift differential for overnights.
  • $2 shift differential for weekends (Friday evening, Saturday, and Sunday).
  • 3 weeks of paid remote training.
