Description

  • Coordinate follow-up, execution, and documentation for patients transitioning into the resupply phase.
  • Partner closely with Sleep Technologists during the transition from treatment setup to long-term resupply.
  • Execute timely and proactive resupply outreach to patients.
  • Follow structured retention workflows to support patient engagement and continuity of care.
  • Maintain accurate reorder tracking and monitor engagement and reorder metrics.
  • Communicate clearly and compassionately with patients throughout the therapy continuity journey.
  • Escalate clinical or billing issues to the appropriate teams.
  • Support adherence monitoring efforts.
  • Collaborate with clinical, billing, and operations teams in a tech-enabled care environment.
  • Help build a scalable operational foundation for a fast-growing digital sleep clinic.

Requirements

  • 2+ years of experience in healthcare coordination, DME, or care navigation.
  • Experience with PAP/CPAP resupply is a strong plus.
  • Strong patient communication skills with empathy and professionalism.
  • Ability to work autonomously in a fast-paced, remote environment while collaborating with cross-functional teams.
  • Confidence navigating digital tools and care platforms.
  • Data-driven mindset with the ability to track tasks, maintain accurate records, and follow structured workflows.
  • Comfort managing proactive outreach and patient follow-up with strong attention to detail.
  • Commitment to improving patient experience, therapy adherence, and continuity of care.
  • Operational and automation mindset.
  • Availability to work Central Time (CST) business hours is preferred.
