Description

  • Deliver coordinated, patient-centered virtual care management by telephone or video to improve members’ health outcomes.
  • Create care plans with members and the care team and support members in achieving their goals.
  • Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system.
  • Partner with members’ local providers to ensure coordinated care.
  • Provide longitudinal follow-up care and build supportive relationships with members and families.
  • Support members and families through acute healthcare episodes such as hospitalizations and rehabilitation stays.
  • Coordinate clinical and social resources to address members’ needs holistically.
  • Gather clinical histories, answer patient-level questions, and summarize findings clearly for internal and external communication.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • 5+ years of nursing experience.
  • 2+ years of care management experience.
  • Must reside in California and hold an active California nursing license.
  • Willingness to become and maintain licensure in multiple states.
  • Experience working with a health plan, health navigator, or third-party administrator (TPA).
  • Experience working remotely and using multiple computer and medical record systems.
  • Comfort discussing multiple medical conditions across a wide age range of populations.
  • Spanish-speaking ability is desirable.
  • Work schedule is 9:00 AM to 6:00 PM PST.
  • Strong empathy, communication, and the ability to work with patients and families during difficult times.
  • Ability to follow security and HIPAA regulations and work efficiently in a high-volume environment.
