Description

  • Deliver coordinated, patient-centered care management by telephone or video to improve members’ health outcomes.
  • Create care plans with members and the clinical care team and support members in achieving their goals.
  • Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system.
  • Partner with members’ local providers to ensure coordinated care.
  • Provide longitudinal follow-up care and build supportive relationships with members and families.
  • Support members and families during acute healthcare episodes such as hospitalizations and rehabilitation stays.
  • Coordinate resources that address members’ clinical and social needs holistically.
  • Gather clinical histories, answer member questions, and summarize findings clearly for the care team.
  • Communicate and collaborate with a diverse clinical team, including care coordinators and records specialists.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • 5+ years of nursing experience.
  • 2+ years of experience working in care management.
  • Must reside in California and hold an active California RN license.
  • Willingness to obtain and maintain licensure in multiple states.
  • Available to work a 9 AM–6 PM PST schedule.
  • Experience working with a health plan, health navigator, or third-party administrator (TPA).
  • Comfort discussing multiple medical conditions and working with populations across age ranges.
  • Experience working remotely and using multiple computer and medical record systems.
  • Spanish-speaking ability is desirable.
  • Strong empathy, communication, and patient-centric writing skills.
  • Ability to follow security and HIPAA regulations strictly.
  • Ability to work promptly and regularly, remain seated for long periods, and use keyboard/computer equipment.
  • Ability to work effectively with leadership, employees, members, and families.
