Description

  • Conduct outreach to patients to offer encouragement, check in on goal progress, and help problem-solve.
  • Provide support from a peer perspective based on first-hand experience with mental health diagnosis and/or addiction.
  • Review patient status and escalate urgent issues to care team providers as needed.
  • Document patient interactions in the patient’s medical record.
  • Coordinate care with the patient’s therapist, prescriber, and other care team members.
  • Work collaboratively with internal and external members of the care team.
  • Perform daily telephonic outreach based on patient needs.
  • Engage with patients via phone and/or video as a role model, mentor, advocate, and motivator to recovery.

Requirements

  • Current and valid New York State Certified Peer Specialist Certification (NYCPS).
  • 1 year of Peer Specialist experience working with adults age 18+ with a mental health diagnosis.
  • Current or former recipient of mental health services.
  • Knowledge of recovery and overcoming challenges related to a mental health diagnosis.
  • Commitment to one’s own continued recovery.
  • Ability to work independently in a virtual environment.
  • Ability to share personal struggles and stories of success.
  • Experience with addiction recovery or substance use support (implied by the role’s peer support focus).
