Description

  • Provide ongoing supervision to the Promotora de Salud team through regular one-on-one check-ins and performance reviews.
  • Monitor team skills, attendance, job functions, and progress toward program goals.
  • Develop and implement tools to document performance and support quality tracking.
  • Ensure timely and complete documentation of team activities and maintain quality assurance measures.
  • Troubleshoot blockers and escalate issues to leadership when appropriate.
  • Support direct reports in administering assessments, identifying member needs, and making appropriate referrals.
  • Facilitate case conferencing to review client cases and provide team support.
  • Serve as an on-call resource to consult with the team as needed.
  • Oversee operational data collection and reporting to ensure quality service delivery.
  • Assist with administrative activities that support community events and program changes.

Requirements

  • Bachelor’s degree or equivalent related experience in the social services field.
  • 3+ years of experience in any healthcare setting.
  • Experience working with Medicaid and/or Medicare.
  • 2+ years of experience as a CHW, PdS, or in a similar role.
  • 2+ years of experience supervising frontline staff.
  • Experience working directly with patients, physicians, physician office staff, or healthcare teams.
  • Experience coordinating care, advocating for, or navigating services on behalf of patients.
  • Ability to lead, make difficult decisions, and remain consistent and calm in challenging situations.
  • Strong supervisory, counseling, teaching, motivating, and team-building skills.
  • Excellent written, verbal, and interpersonal communication skills, including strong listening skills.
  • Strong computer proficiency and computer literacy.
  • Ability to maintain HIPAA confidentiality and security requirements.
  • Fluency in English and Spanish, both verbal and written.
  • Must be authorized to work in the United States.
  • Must reside in Santa Clara, Santa Cruz, or San Francisco counties.
  • Experience working within the community and a strong service-minded orientation.
  • Preferred: proactive learner comfortable working in different environments.
