• Serve as the primary point of contact for assigned members and build trust through personalized, high-touch support.
• Help members understand and use the Twin platform to make informed lifestyle changes.
• Conduct structured member conversations at a defined frequency to motivate progress and support better health outcomes.
• Provide individualized nutrition and lifestyle coaching focused on chronic metabolic disease remission and overall health improvement.
• Interpret and review member health data daily to identify timely, actionable interventions.
• Encourage sustained behavior change through education, motivation, and ongoing support.
• Own member adherence, retention, and reactivation by using early warning signals to prevent drop-offs.
• Handle exceptions and escalations calmly and professionally while providing solutions and emotional support.
• Monitor key performance metrics, including engagement, retention, nutrition adherence, health outcomes, and referrals.
• Collaborate with Doctors, Coaches, and Customer Success to ensure coordinated care delivery and maintain SOP compliance.
