Description

  • Provide care, disease management, and health education to patients.
  • Support individual patient goal-setting asynchronously to help members manage chronic conditions.
  • Create personalized action plans and behavior change plans for patients.
  • Conduct regular outreach to patients based on their needs and preferences to support engagement.
  • Coordinate with other clinical team members to deliver an exceptional patient experience.
  • Develop and maintain supportive, professional relationships with patients and team members.
  • Create and distribute health education materials for individual members as needed.
  • Work with the cross-functional product team to improve the in-app patient experience.
  • Assist with medical and administrative tasks to keep members moving through the program.

Requirements

  • 2+ years of experience as a Licensed Practical Nurse (LPN/LVN) or other nursing license.
  • 2+ years of experience in patient-facing or customer-facing roles.
  • Active compact state license required; additional licensing may be needed.
  • Bilingual Spanish-speaking ability is a plus.
  • Excellent customer relations, written communication, and verbal communication skills.
  • Knowledge of medical terminology and general medical office procedures.
  • Familiarity with digital tools such as Slack, Coda, and Google Workspace.
  • Strong analytical and proactive problem-solving skills.
  • Self-motivated, results-oriented, and strategic mindset.
  • Must be authorized to work in the United States.
  • Experience in telehealth or a healthcare startup environment preferred.
  • Experience in GI, weight management, or with anti-obesity medications such as GLP-1s is a bonus.
