Description

  • Own the end-to-end voice AI architecture from telephony streams through LLM orchestration to text-to-speech output and call disposition.
  • Design, build, and operate a production voice AI system for high-volume patient interactions.
  • Implement multi-agent conversational workflows with tool calling, agent handoffs, and shared conversation state.
  • Build and optimize real-time audio pipelines, including WebSocket streaming, codec handling, VAD configuration, and interruption management.
  • Develop analytics and observability for voice-specific metrics such as latency, call efficacy, disposition accuracy, and ASR error rates.
  • Solve voice-specific challenges including turn-taking, silence detection, barge-in recovery, medical term recognition, and latency optimization.
  • Integrate voice agents with internal services through secure authenticated APIs.
  • Improve platform reliability by eliminating single points of failure, adding multi-provider LLM failover, and designing graceful degradation paths.
  • Collaborate with product and clinical operations to improve self-serve outcomes and reduce escalations.
  • Mentor team members on voice AI best practices and contribute to architectural decisions.

Requirements

  • 5+ years of software engineering experience, including at least 2 years building production voice AI or conversational AI systems.
  • Deep experience with end-to-end voice AI pipelines from telephony through STT, LLM processing, and TTS.
  • Production experience with agentic architectures, including multi-agent orchestration, tool calling, agent handoffs, memory/state management, and real-time decision making.
  • Strong understanding of voice-specific challenges such as VAD tuning, turn-taking, interruption/barge-in handling, latency budgets, and audio codec management.
  • Hands-on experience with telephony systems such as Twilio media streams, SIP, IVR, or equivalent WebSocket-based audio streaming platforms.
  • Proficiency in TypeScript/Node.js with strong async programming patterns; experience with NestJS or similar frameworks.
  • Experience with STT/TTS providers such as Deepgram, OpenAI, ElevenLabs, or Azure Speech.
  • Production experience with LLM APIs such as OpenAI Realtime API, Anthropic Claude, or equivalent, plus prompt engineering for conversational agents.
  • High agency and autonomy, with the ability to self-unblock and drive work to completion.
  • Excellent communication skills for explaining complex voice AI decisions to product and clinical stakeholders.
  • Experience in healthcare, biotech, or regulated environments such as HIPAA, PHI handling, zero-retention architectures, or BAA compliance.
  • AWS infrastructure experience with ECS Fargate, Lambda, DynamoDB, Bedrock, Kafka/MSK, API Gateway, or CDK.
  • Background in real-time systems, including WebSocket lifecycle management, connection resilience, and streaming protocols.
  • Experience building analytics pipelines for voice or conversational metrics.
  • Familiarity with RAG architectures, including vector stores, embedding models, and chunking strategies.
  • Track record of migrating or evaluating vendor platforms while maintaining production uptime.
  • Experience with Datadog APM, LLM observability, or equivalent monitoring tools for AI systems.
  • Prior experience in a high-growth startup or zero-to-one product environment.
