Description

  • Build and maintain relationships with community stakeholders such as social workers, discharge planners, senior centers, nonprofits, and other local organizations.
  • Promote Mom’s Meals through in-person outreach, community events, presentations, and educational sessions.
  • Increase awareness of the Michigan ILOS program and other meal-benefit programs available to eligible residents.
  • Identify and engage eligible clients and help them understand their options and complete the enrollment process.
  • Serve as a liaison between clients, referral sources, and internal teams to support seamless service delivery.
  • Track outreach activities, client engagement, and outcomes using CRM tools or internal systems.
  • Stay current on local and state programs that offer meal benefits and align outreach efforts accordingly.
  • Represent Mom’s Meals at fairs, expos, conferences, coalition meetings, county roundtables, and webinars.
  • Provide field feedback to help improve customer experience and reduce barriers to access.
  • Cover for the Manager of Healthcare Partnerships when needed.

Requirements

  • Associate’s or Bachelor’s degree.
  • 2+ years of experience in community outreach, sales, or healthcare.
  • Demonstrated ability to engage community health organizations successfully.
  • Proven ability to work with cross-functional teams.
  • Excellent oral and written communication skills, including presentation experience.
  • Experience using Microsoft Office, including PowerPoint and Excel.
  • Willingness to travel locally and statewide примерно 75% of the time, mostly during daytime hours, with 1-2 overnight stays as needed and two national sales meetings per year.
  • Experience in social services or healthcare is preferred.
  • Strong interpersonal skills and a customer-focused approach are preferred.
  • Ability to work independently in the field with minimal supervision is preferred.
  • Passion for serving underserved or vulnerable populations is preferred.
  • Knowledge of Medicaid, Medicare, and social assistance programs is preferred.
  • Valid driver’s license and reliable transportation.
