Description

  • Provide telephone triage for hospice patients and their families after hours.
  • Assess patient conditions and determine the appropriate next steps for care.
  • Coordinate with on-call teams to support timely interventions and care resources.
  • Document calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, and empathetic demeanor during urgent calls.
  • Support patient-centered care for hospice and home health clients during non-core hours.
  • Work independently while staying in active communication with the care team.
  • Help ensure timely coordination of care for patients receiving hospice services.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to handle stress and multitask while managing a high call volume.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts.
  • Must be able to support approximately 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

Benefits

  • Base pay of $25 per hour, with potential to earn up to $28 per hour within the first 6 months.
  • $2 shift differential for overnight shifts and $2 shift differential for weekends.
