Description

  • Provide telephone triage for hospice patients and their families after hours.
  • Assess patient conditions and determine appropriate next steps for care.
  • Coordinate with on-call teams to arrange care and necessary resources.
  • Document all calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, compassionate demeanor when handling urgent calls.
  • Support timely patient care coordination for hospice services during non-core hours.
  • Work independently while communicating consistently with the remote clinical team.

Requirements

  • Active multistate Licensed Practical Nurse (LPN) license.
  • Hospice, palliative, or end-of-life care experience is strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for call documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to handle stress, multitask, and manage a high call volume of at least 5 calls per hour on weekdays and up to 8 per hour on weekends.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Reliable internet connectivity and consistent attendance during all scheduled shifts are required.

Benefits

  • Base pay of $19 per hour after training, with opportunities to increase to up to $22 per hour within the first 6 months.
  • $2 shift differential for overnight shifts.
  • $2 shift differential for weekend shifts (Friday evening, Saturday, and Sunday).
  • 3 weeks of paid remote training.
