Description

  • Deliver coordinated, patient-centered virtual care management by telephone or video to improve members' health outcomes.
  • Create care plans with members and the clinical care team and help members achieve their goals.
  • Guide members through complex medical conditions, treatment pathways, benefits, and the healthcare system.
  • Partner with local providers to support coordinated care.
  • Provide longitudinal follow-up and build supportive relationships with members and their families.
  • Support members and families during acute episodes such as hospitalizations and rehabilitation stays.
  • Coordinate clinical and social resources to address members' holistic needs.
  • Gather clinical histories, answer member questions, and summarize findings clearly for the care team.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • Current CCM certification or eligibility for the exam in August 2026.
  • 5+ years of nursing experience.
  • 2+ years of experience in care management.
  • Must reside in a compact NLC state.
  • Active Compact RN license in good standing.
  • Active California nursing license.
  • Willingness to obtain and maintain licensure in multiple states.
  • Ability to work until 6pm PST; preference for candidates in MST/PST time zones.
  • Experience working with a health plan, health navigator, or third-party administrator (TPA).
  • Experience working remotely and using multiple computer and medical record systems.
  • Spanish-speaking ability is desirable.
  • Comfort discussing a range of medical conditions across age groups.
  • Ability to write medical information in clear, patient-centric language.
  • Strong empathy, responsiveness, and ability to work within HIPAA and security requirements.
