Description

  • Work directly with patients, caregivers, and providers in Empassion’s palliative care program.
  • Help patients understand the value of palliative care and enroll them in the program.
  • Connect patients with palliative care providers and coordinate care follow-up.
  • Answer questions from patients, caregivers, and providers about the program and palliative care.
  • Document patient encounters and triage patient needs to the appropriate care team.
  • Follow up with patients to ensure they are receiving high-quality care from Empassion’s network of providers.
  • Collaborate with the team on process improvement.
  • Serve as a trusted resource for patients and their families.
  • Communicate regularly with the Central Operations team via Zoom and Slack.

Requirements

  • Minimum 2+ years of relevant work experience in a customer service call center or healthcare setting.
  • Basic understanding of healthcare.
  • Caring and compassionate attitude; social work or other people-centered background preferred.
  • Strong communication skills and comfort speaking with patients on the phone.
  • Ability to work comfortably with technology in a dynamic startup environment.
  • Empathy and a patient-focused mindset.
  • Bilingual in Spanish and English is a plus but not required.
  • Secure Wi-Fi connectivity.
  • Ability to multitask effectively in a remote work environment.
