Description

  • Organize and maintain case files, records, service plans, and related documentation.
  • Schedule and coordinate appointments, referrals, service authorizations, annual reviews, and follow-ups.
  • Serve as a liaison between clients, families, service providers, and community organizations.
  • Assist in developing, monitoring, and updating individualized service plans.
  • Track deadlines, renewals, and case progress to keep the team informed.
  • Monitor compliance with state and agency documentation and service delivery requirements.
  • Collaborate with healthcare providers, therapists, and service coordinators to support person-centered care.
  • Prepare and submit reports on client progress, service utilization, and outcomes.
  • Support case managers in resolving care-related concerns and access barriers.
  • Provide interpretation and translation support for Spanish-speaking clients and families.

Requirements

  • 2 years of experience as a Case Management Assistant or in a similar administrative role.
  • Experience with scheduling, documentation, reporting, and coordination tasks in healthcare or social services.
  • Bilingual Spanish and English required, both written and verbal.
  • Experience working with US-based clients required.
  • Strong organizational skills with meticulous attention to detail.
  • Proficiency in case management software, Microsoft Office, or similar tools.
  • Excellent interpersonal and communication skills with a client-first mindset.
  • Ability to maintain strict confidentiality and handle sensitive information professionally.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Experience in a health or human services setting is strongly preferred.
