Description

  • Provide evidence-based behavioral health interventions to pediatric and/or adult patients with mild to moderate symptoms using the Collaborative Care Model.
  • Oversee up to 15 Collaborative Care Clinicians, including clinical metrics, schedule management, coaching, performance management, and supervision.
  • Support pod and partner workflows in collaboration with the Clinical Excellence Manager.
  • Manage employee schedules, shift coverage, time-off approvals, and attendance policy adherence.
  • Supervise and mentor staff on patient care, performance evaluations, career development, and disciplinary actions.
  • Resolve and document complex patient situations, escalated complaints, patient terminations, and incident reports.
  • Coordinate care with primary care providers, psychiatric consultants, and other service providers.
  • Screen and assess patients using behavioral health and substance use tools such as PHQ9, GAD7, and Columbia Suicide Risk Assessment.
  • Monitor symptoms, medication side effects, follow-up, and clinical outcomes through registries and the EHR.
  • Document patient progress, treatment recommendations, and care plans in Salesforce and the electronic health record.

Requirements

  • Master’s degree in social work, counseling, or a related field.
  • Active California license as an independent clinician (LCSW, LPCC, or equivalent).
  • 5+ years of clinical experience.
  • 3+ years of lead or supervisory experience.
  • Proficiency in evidence-based approaches including Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation.
  • Technical proficiency with EHRs and ability to quickly learn new technology.
  • Knowledge of or experience with PHQ9, GAD7, CSSRS, and similar behavioral health tools.
  • Ability to provide clinical care in primary care settings for patients as young as 6 years old, including women’s health populations.
  • Knowledge of CoCM.
  • Fluency in English and Spanish preferred.
