Description

  • Coordinate and complete Health Risk Assessments (HRAs) through telephone outreach.
  • Collect, review, and document health history, medications, chronic conditions, social determinants of health, and preventive care needs.
  • Maintain accurate and timely documentation in electronic health record (EHR) systems and internal databases.
  • Support care coordination by scheduling Annual Wellness Visits (AWVs).
  • Monitor HRA completion metrics, productivity goals, and quality standards, including an expectation of 7 HRAs per day.
  • Ensure compliance with HIPAA, CMS, Medicare, and organizational policies and procedures.
  • Collaborate with nurses, providers, case managers, and quality improvement staff.
  • Educate patients and members on healthcare resources, wellness programs, and preventive services.

Requirements

  • Active, unencumbered clinical license or certification such as LPN, RN, CMA, or RMA.
  • Active Basic Life Support (BLS) certification.
  • Bilingual in English and Spanish.
  • Proven ability to manage high-volume telephonic outreach and meet daily productivity goals, such as 7 HRAs per day.
  • Strong knowledge of health risk factors, chronic conditions, and social determinants of health (SDOH).
  • High proficiency with Electronic Health Record (EHR) systems and accurate, timely documentation.
  • Excellent organizational, time management, and critical thinking skills.
  • Thorough understanding of HIPAA, CMS, and Medicare compliance standards.
  • Excellent verbal communication skills for patient education and collaboration with clinical teams.
