Description

  • Serve as the primary point of contact for enrolled patients and build trusting relationships over time.
  • Conduct regular check-ins to assess patient health status, care needs, and social barriers.
  • Manage a caseload of approximately 150 patients and prioritize outreach based on acuity, transitions, and care gaps.
  • Coordinate appointments, facilitate communication across providers, and help ensure care plans are understood and actionable.
  • Support medication adherence by identifying barriers, educating patients, and escalating concerns to clinical staff.
  • Connect patients with durable medical equipment, transportation, meal programs, benefits, and community resources.
  • Perform structured social determinants of health screenings and follow up on resource access.
  • Provide condition-specific education and self-management support tailored to patient needs and literacy levels.
  • Complete timely post-hospital and emergency department follow-up calls and reconcile discharge instructions and medications.
  • Document all interactions, track navigation time for billing, and maintain compliance with CMS, HIPAA, and program protocols.

Requirements

  • Authorized to work in the US without employee sponsorship.
  • Must be located in Florida, Georgia, North Carolina, Tennessee, or Texas.
  • If located in Texas, must hold a current, active CHW certification.
  • Active Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential.
  • 1+ years of patient-facing healthcare experience in medical front office, ambulatory care, primary care, senior care, case management, patient coordination, utilization management, or value-based care.
  • Experience working with older adults and individuals managing serious chronic conditions.
  • Strong understanding of care coordination principles and healthcare workflows.
  • Comfortable discussing chronic conditions, medications, and treatment plans without providing medical advice.
  • Proficient with EHR systems, care management platforms, and digital communication tools.
  • High school diploma or equivalent required; associate's or bachelor's degree in healthcare, social work, public health, or related field strongly preferred.
  • Proven remote work capability with reliable internet and a professional home workspace.
  • Community Health Worker (CHW) certification or training preferred.
  • Experience with Medicare-enrolled or dual-eligible populations preferred.
  • Familiarity with value-based care models, ACOs, or Medicare Advantage programs preferred.
  • Experience with post-hospital or post-ED transitional care calls preferred.
  • Prior experience with SDOH screening tools or community resource navigation preferred.
  • Bilingual ability, especially Spanish, strongly preferred.
  • Experience with common care management EHR platforms such as Epic, Cerner, or Allscripts preferred.
