Description

  • Manage patient census with a resolution-focused approach to close gaps in clinical and non-clinical care.
  • Identify and coordinate community resources to support patients' care needs.
  • Provide patient education and health literacy support for chronic condition management.
  • Perform medication management, including reconciliation, adherence support, refill coordination, and identifying potential concerns.
  • Coordinate timely delivery of services such as home health, durable medical equipment, home infusion, and other patient needs.
  • Respond to patient questions and maintain open communication to understand care needs.
  • Assist with referrals and appointment scheduling.
  • Work closely with providers to deliver value-based care management initiatives.

Requirements

  • Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license with Compact/Multi-State eligibility.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.
  • Sedentary role requiring prolonged sitting and extended telephone-based communication.
  • Ability to understand, process, and take thorough real-time notes during phone conversations.
  • Epic experience is preferred.
  • Bilingual skills are preferred.
  • Additional single-state licensure is preferred.
