Description

  • Implement products and execute ACO initiatives under the direction of Market Leadership.
  • Support wellness visits, transitions of care, and active patient management.
  • Ensure practices meet goals outlined in mutually developed Practice Transformation Plans.
  • Assist in training office staff, physicians, and providers on processes and technologies.
  • Coordinate training using Aledade's Value Based Care Curriculum.
  • Continuously assess practice and staff training needs.
  • Build and maintain relationships with physicians, providers, and healthcare system executives.
  • Support financial and quality performance across practice performance metrics.
  • Listen to practice concerns and relay feedback to managers and leads to identify improvement opportunities.

Requirements

  • Must be a resident of Tennessee and be based in Tennessee.
  • Graduate of an accredited college or university.
  • 4-6 years of relevant work experience in medical practice management may be accepted in lieu of educational requirements, especially with significant clinic administrative experience.
  • 3 years of experience with Electronic Health Records (EHR) for clinical or practice management processes.
  • Excellent oral and written communication skills, including public presentation skills.
  • Strong organizational and project management skills.
  • Team-building skills and ability to engage stakeholders effectively.
  • Familiarity with process mapping and workflow analysis tools.
  • Excellent computer skills and willingness to learn additional software applications.
  • Ability to travel to practices up to 25% of the year across Tennessee.
