Description

  • Answer inbound and outbound calls after hours, including evenings, nights, weekends, and holidays, within scope of practice.
  • Consult with on-call providers and other clinical staff to support participant care decisions.
  • Coordinate telehealth meetings between participants and clinicians.
  • Resolve logistical care-delivery issues related to transportation, medication delivery, and hospital discharge.
  • Support care transitions between hospitals, skilled nursing facilities, participants’ homes, and other care settings.
  • Escalate changes in participants’ conditions and unresolved issues to management or the appropriate clinical team.
  • Provide continuity of care when the PACE center is closed and respond to urgent participant needs.
  • Visit participants’ residences or acute care settings as needed to address issues within scope.

Requirements

  • Graduate of an accredited school of nursing with an unencumbered RN license.
  • Experience or strong knowledge in triaging is ideally preferred.
  • Clinical knowledge and skills necessary to care for frail, elderly participants and manage complex clinical situations.
  • Ability to work a 20-hour-per-week schedule with assigned shifts that may include days, evenings, nights, weekends, holidays, and overtime.
  • Highly motivated and self-directed with the ability to make sound decisions in emergency situations.
  • Excellent clinical, organizational, and communication skills when working with seniors, families, and interdisciplinary teams.
  • Ability to execute tasks effectively in a quickly changing environment.
  • Understanding of or experience in holistic, value-based, community-based care for older adults (preferred).
