Description

  • Administer multiple assessments and screening tools across the care timeline.
  • Interpret assessment results and help prioritize member needs.
  • Develop individualized care plans within the multidisciplinary care team.
  • Lead care coordination efforts through internal and joint rounds.
  • Use Tuesday Health’s electronic systems and tools to support coordinated care delivery.
  • Build trusting, empathetic relationships with members and their caregivers.
  • Serve as the central coordinator for the multidisciplinary team supporting members with serious illness.

Requirements

  • Active and unrestricted Registered Nurse license in the State of Ohio with no board action.
  • Must be located in Ohio and able to work remotely.
  • Experience in a clinical or medical setting preferred.
  • Experience working in a multidisciplinary care model across network providers, health plan care management support, and employed clinicians.
  • Strong communication, organization, and written skills.
  • Ability to multitask and work under pressure while maintaining a positive member experience.
  • Ability to recognize when to escalate and respond professionally in emergent situations.
  • Strong technical skills with documentation systems and tools, plus the ability to learn new systems as the company grows.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid driver’s license, safe driving record, and state minimum automobile insurance coverage.
  • Comfort with change and ambiguity in a rapidly evolving business.
  • Bilingual Spanish preferred.
