Description

  • Serve as the primary point of contact for patients with neurodegenerative diseases and their caregivers.
  • Build and maintain compassionate relationships that support the patient care experience.
  • Execute patient care plans and coordinate connections to neurology experts, genetic experts, and referral resources.
  • Provide basic health education about diagnosis, treatment and care options, and research opportunities.
  • Maintain high standards of privacy, confidentiality, and quality in client and partner interactions.
  • Develop and follow standardized operating procedures, quality assurance practices, and documentation workflows.
  • Collaborate with the Care Coordination team and broader Synapticure organization to create a streamlined patient experience.
  • Work with internal teams, medical partners, and community partners to represent the program effectively.
  • Maintain working knowledge of research developments and trends in neurodegenerative diagnoses, treatment, and care.
  • Communicate clearly and professionally about the program internally and externally.

Requirements

  • Bachelor’s degree in a related field.
  • 2+ years of experience in direct case or care management in a healthcare setting.
  • Comfortable supporting members remotely using telephone, text, and virtual visit platforms.
  • Excellent verbal, written, organizational, and interpersonal communication skills.
  • Experience using scheduling platforms for accurate appointment scheduling and management.
  • Experience using electronic health record systems and/or care facilitation platforms for accurate documentation.
  • Ability to collect member clinical and demographic data and document it accurately and in a timely manner.
  • Strong problem-solving ability, including knowing when to make decisions independently and when to collaborate.
  • Ability to provide creative solutions to healthcare system barriers affecting patient care.
  • Growth mindset, adaptability, and comfort with fast-paced change and shifting priorities.
  • Ability to build cooperative working relationships with patients, teammates, and healthcare and community service providers.
  • Experience with clinical care of patients with neurodegenerative diseases (preferred).
  • Bilingual Spanish verbal and written fluency to support a diverse patient population (preferred).
  • Fully remote role with occasional travel to headquarters in Chicago, IL and/or other locations may be expected.
