Description

  • Locate and engage eligible patients using telephonic, written, and digital outreach methods.
  • Leverage EMR data to research potential patients and inform outreach strategies.
  • Complete patient intake and consent processes and transition fully enrolled patients to a Care Coordinator.
  • Serve as the patient’s initial point-of-contact, building trust and providing support, education, and a listening ear prior to full enrollment.
  • Conduct high-volume outbound calls and execute creative outreach campaigns to meet outreach goals.
  • Document patient information, track tasks, and communicate with staff, patients, and family members daily within the EMR.
  • Provide product and process feedback to the product development team and participate as a super-user to assist onboarding and support colleagues.

Requirements

  • 0–2 years of customer service or sales experience with a successful track record.
  • Passion for working with your community or communities you have been part of.
  • Strong interpersonal skills with empathy, curiosity, and the ability to connect authentically with patients and families.
  • Metrics-driven mindset and willingness to meet quotas for the Outreach team.
  • Comfort with high-volume outbound calling and creative outreach approaches.
  • Independent self-starter who adapts well to change and shifting priorities.
  • Comfortable and confident using technology to support work and improve processes, including working in an EMR.
  • Ability to work remotely from home; occasional travel to Synapticure headquarters in Chicago, IL may be required.
