Description

  • Process, track, and coordinate patient referrals to ensure timely appointment scheduling.
  • Schedule PCP, dental, vision, specialist, and transportation appointments for patients.
  • Verify patient insurance eligibility and confirm in-network primary care providers.
  • Coordinate Release of Information (ROI) and Authorized Representative (AR) requests.
  • Maintain accurate documentation of referral activity and communications in internal systems.
  • Collaborate with case managers, providers, and health plans to support continuity of care.
  • Support smooth transitions between services and a positive patient experience.
  • Contribute to process improvements in referral workflows and scheduling efficiency.

Requirements

  • Fluency in English and Spanish.
  • 1+ years of healthcare operations or administrative experience in a fast-paced healthcare environment.
  • Experience as a Referral Coordinator, Medical Assistant, or in healthcare appointment scheduling preferred.
  • Strong organizational skills and attention to detail with the ability to manage multiple tasks and systems simultaneously.
  • Comfort navigating insurance portals and verifying eligibility or provider information.
  • Excellent written and verbal communication skills with professionalism and empathy.
  • Proficiency with Google Suite, Slack, and data tools such as Looker or comparable platforms.
  • Reliable, HIPAA-compliant workspace with a stable internet connection.
  • Commitment to improving healthcare access for underserved communities.
  • Must be based in the United States and able to work PST hours.
