Description

  • Deliver coordinated, patient-centered virtual care management by telephone or video to improve member health outcomes.
  • Create and update individualized care plans with members and the care team to help members achieve their goals.
  • Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system.
  • Partner with local providers to support coordinated care across settings.
  • Provide compassionate longitudinal follow-up and build supportive member relationships over time.
  • Support members and families during acute healthcare episodes such as hospitalizations and rehabilitation stays.
  • Coordinate clinical and social resources that address members’ holistic needs.
  • Guide members through complex behavioral health, psychiatric, and substance use situations.
  • Communicate clinical histories, answer questions, and summarize findings in patient-friendly language.

Requirements

  • Bachelor of Science in Nursing (BSN).
  • Current CCM certification required.
  • 5+ years of nursing experience.
  • 2+ years of experience in care, case, and disease management, preferably in a health plan, health navigator, or TPA environment.
  • 2+ years of experience working in behavioral health and/or substance use.
  • Active Compact RN license in good standing.
  • Must reside in a compact NLC state.
  • Active California nursing license preferred.
  • Willingness to obtain and maintain licensure in multiple states.
  • Ability to work until 9-6 PM PST.
  • Spanish speaking desirable.
  • Experience working remotely and using multiple computer and medical record systems.
  • Comfort discussing multiple medical conditions and working with populations across age ranges.
  • Strong empathy, communication, and ability to work collaboratively while following HIPAA and security regulations.
