Description

Deliver coordinated, patient-centered virtual care management by telephone or video to improve member health outcomes.
Create and update individualized care plans with members and the care team to help members achieve their goals.
Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system.
Partner with local providers to support coordinated care across settings.
Provide compassionate longitudinal follow-up and build supportive member relationships over time.
Support members and families during acute healthcare episodes such as hospitalizations and rehabilitation stays.
Coordinate clinical and social resources that address members’ holistic needs.
Guide members through complex behavioral health, psychiatric, and substance use situations.
Communicate clinical histories, answer questions, and summarize findings in patient-friendly language.

Requirements

Bachelor of Science in Nursing (BSN).
Current CCM certification required.
5+ years of nursing experience.
2+ years of experience in care, case, and disease management, preferably in a health plan, health navigator, or TPA environment.
2+ years of experience working in behavioral health and/or substance use.
Active Compact RN license in good standing.
Must reside in a compact NLC state.
Active California nursing license preferred.
Willingness to obtain and maintain licensure in multiple states.
Ability to work until 9-6 PM PST.
Spanish speaking desirable.
Experience working remotely and using multiple computer and medical record systems.
Comfort discussing multiple medical conditions and working with populations across age ranges.
Strong empathy, communication, and ability to work collaboratively while following HIPAA and security regulations.
