Description

  • Conduct telehealth patient assessments, including history reviews, physical exams, and diagnostic test evaluation.
  • Formulate differential diagnoses and develop evidence-based treatment plans for cardiovascular patients.
  • Prescribe medications and order, interpret, and act on diagnostic tests as needed.
  • Manage chronic cardiovascular conditions remotely, including hypertension, heart failure, arrhythmias, and coronary artery disease.
  • Educate patients and families on prevention, lifestyle changes, medication management, diet, exercise, and smoking cessation.
  • Collaborate with cardiologists, primary care providers, APPs, and cross-functional teams to coordinate care.
  • Lead clinical quality initiatives, including peer review, chart review, and quality oversight of clinicians.
  • Develop, implement, and improve clinical quality programs, performance standards, and evidence-based protocols.
  • Design and deliver onboarding, training, and ongoing education for clinicians.
  • Serve as a clinical resource and mentor while providing coaching, feedback, and support for clinician development.

Requirements

  • Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
  • Current, unrestricted Nurse Practitioner/APRN license.
  • Active national board certification such as AANP or ANCC.
  • Eligibility to obtain and maintain licensure in states required by the organization.
  • Strong clinical experience as a Nurse Practitioner managing cardiovascular patients.
  • Cardiology experience strongly preferred.
  • Experience with clinical quality initiatives, chart review, peer review, quality assurance, or performance improvement.
  • Experience developing or delivering clinical education, onboarding, training, or ongoing professional development.
  • Ability to independently assess patients, create differential diagnoses, prescribe medications, and manage chronic cardiovascular conditions.
  • Comfort delivering patient care through telehealth and collaborating effectively with clinical teams.
  • Commitment to ongoing continuing education and maintaining required credentials.
