Description

  • Onboard and guide caregivers and patients in the platform.
  • Maintain patient and caregiver profiles with up-to-date information.
  • Assess needs and provide empathetic, patient-centered support for people with dementia and their caregivers.
  • Serve as the primary point of contact and advocate for timely issue resolution.
  • Coordinate solutions with clinical teams and community resources.
  • Connect patients with community resources and facilitate clinical referrals.
  • Support home device setup and usage.
  • Analyze in-home system data and activate established clinical protocols when needed.
  • Document all information exclusively in the platform while ensuring HIPAA compliance.
  • Report incidents, support process improvements, and contribute to system validation.

Requirements

  • Degree or training in Nursing or Social Work.
  • Previous experience as a caregiver, preferably with people living with dementia.
  • Experience serving U.S. populations and knowledge of the U.S. healthcare and insurance system.
  • 3 years of experience working with patients in clinics, customer service, or with clinical teams.
  • Advanced English proficiency.
  • Strong organizational, communication, and time management skills.
  • Basic ability to use an AI-based platform and SAS systems.
  • Experience with research and information-gathering tools such as ChatGPT and Gemini.
  • Strict knowledge of and compliance with HIPAA.
  • Basic digital literacy and the ability to document accurately.
  • Knowledge of medical terminology.
