Description

  • Review and assess patient information remotely to monitor medical history, symptoms, medication adherence, and treatment progress, and escalate findings to providers as appropriate.
  • Participate in patient assessments through chart review, telehealth interactions, and standardized assessment tools following established clinical protocols.
  • Assist in updating individualized care plans in coordination with healthcare providers.
  • Review and manage medication lists and support medication refill coordination by routing requests to providers or pharmacies.
  • Maintain accurate and up-to-date patient records in electronic health record (EHR) systems.
  • Communicate with Spanish-speaking patients and families to support care coordination, reminders, education, and follow-ups.
  • Report significant changes in patient condition to the healthcare team and document accordingly.
  • Support training and guidance for staff by assisting with onboarding, reviewing workflows, and reinforcing clinical and documentation standards as directed.
  • Utilize telehealth platforms and secure communication tools for virtual interactions.
  • Ensure HIPAA compliance and maintain patient confidentiality at all times.
  • Organize and manage healthcare documentation, schedules, and records.
  • Support general administrative tasks related to clinical coordination and daily operations as needed.

Requirements

  • Active US Registered Nurse (USRN) license, with Texas license required.
  • Conversational Spanish proficiency (spoken) required, with ability to understand and respond in common patient-care scenarios.
  • Strong understanding of medical terminology and care coordination workflows.
  • High attention to detail when handling healthcare documentation and patient records.
  • Clear verbal and written communication skills with patients, families, and healthcare teams.
  • Proficiency with telehealth platforms and virtual communication tools.
  • Strong organizational and time-management skills.
  • Working knowledge of HIPAA and patient confidentiality requirements.
  • Experience in a remote, virtual, or healthcare support role preferred.
  • Experience using Electronic Health Record (EHR) systems such as Epic, Cerner, or similar.
  • Experience supporting medication management processes and patient coordination.
  • Familiarity with scheduling tools and secure messaging platforms is a plus.
