Description

  • Conduct professional and compassionate outreach to patients by phone to introduce CCM and RPM programs.
  • Evaluate patient eligibility and explain program benefits and expectations clearly.
  • Document verbal consent and patient interactions accurately in the EMR.
  • Follow CMS and HIPAA guidelines in all patient communications and documentation.
  • Collaborate with internal care teams to ensure smooth hand-offs and consistent patient support.
  • Maintain high-quality documentation for care planning and billing purposes.
  • Manage the daily patient queue independently and ensure timely follow-up.
  • Build trust with patients and support enrollment workflow efficiency and program participation.

Requirements

  • Active, nationally recognized Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification.
  • Must live in either MST or PST to be considered.
  • Bilingual fluency in English and Spanish.
  • Current BLS certification.
  • Direct hands-on experience with CCM and/or RPM programs.
  • Strong proficiency in Google Suite and electronic medical record (EMR) systems.
  • Ability to work independently in a secure, HIPAA-compliant home office.
  • Excellent verbal and written communication skills with the ability to explain complex information simply.
  • In-depth knowledge of CMS regulations and documentation requirements for chronic care (preferred).
  • Previous experience in care coordination or transitional care (preferred).
  • Familiarity with clinical quality measures and reporting (preferred).
