Description

  • Manage the patient census and resolve gaps in clinical and non-clinical care.
  • Identify and coordinate community resources that support patient care needs.
  • Provide patient education and health literacy support for chronic condition management.
  • Perform medication management, including reconciliation, adherence support, refill coordination, and identification of medication concerns.
  • Coordinate timely delivery of services such as home health, durable medical equipment, and home infusion.
  • Answer patient questions and maintain open communication to understand patient needs.
  • Assist with referrals and appointment scheduling.
  • Support chronic care management patients through coordinated, patient-facing care activities.

Requirements

  • Active Massachusetts LPN license.
  • At least 3 years of direct patient-facing work experience.
  • Experience identifying resources and coordinating needs for chronic care management patients.
  • Technical aptitude with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi for remote work.
  • Must have a home office or HIPAA-compliant workspace.
  • Epic experience is preferred.
  • Bilingual ability is preferred.
  • Additional state LPN licensure is preferred.
  • Sedentary role requiring prolonged sitting and extended phone-based communication.
  • Ability to speak clearly, listen attentively, and take thorough real-time notes during telephone conversations.
