Description

  • Accept transfers from the Patient Enrollment team and conduct preliminary health assessments for newly enrolled patients.
  • Resolve patient questions and maintain open dialogue to understand needs and concerns.
  • Identify and coordinate community resources that support patient care.
  • Provide patient education and health literacy support for chronic condition management.
  • Assist with medication management, including identifying concerns, supporting adherence, and coordinating refills.
  • Coordinate timely delivery of patient services such as home health, durable medical equipment, home infusion, and other critical needs.
  • Work primarily by phone and adapt quickly to a fast-paced environment.
  • Take thorough real-time notes during telephone conversations and manage patient information accurately.

Requirements

  • Active Compact or Multi-State LPN/LVN license.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.
  • Experience with Epic is preferred.
  • Bilingual ability is a plus.
  • Additional single-state licensures are a plus.
  • Sedentary role requiring prolonged sitting and extended phone-based communication.
