Description

Accept transfers from the Patient Enrollment team and conduct preliminary health assessments for newly enrolled patients.
Resolve patient questions and maintain open communication to understand patient needs.
Identify and coordinate community resources that can support patient care.
Provide patient education and health literacy guidance for chronic condition management.
Assist with medication management, including identifying concerns, supporting adherence, and coordinating refills.
Help ensure timely delivery of services such as Home Health, DME, Home Infusion, and other patient needs.
Work primarily by phone and adapt quickly in a fast-paced environment.
Take thorough real-time notes and document telephone conversations accurately.

Requirements

Active Compact or Multi-State LPN/LVN license.
At least 3 years of direct patient-facing work experience.
Technical aptitude with Microsoft Office Suite.
Excellent written and verbal communication skills.
Must have active high-speed Wi-Fi.
Must have a home office or HIPAA-compliant workspace.
Experience with Epic is preferred.
Bilingual candidates are preferred.
Additional single-state licensures are preferred.
Ability to sit for long periods and speak/listen clearly by telephone for extended periods.
