Description

  • Manage the patient census and take a resolution-driven approach to close gaps in clinical and non-clinical care.
  • Identify and coordinate community resources that can support patients’ care needs.
  • Provide patient education and health literacy support for chronic condition management.
  • Perform medication management, including reconciliation, adherence support, refill coordination, and identifying potential medication concerns.
  • Coordinate timely delivery of services such as home health, DME, home infusion, and other critical needs.
  • Resolve patient questions and maintain open communication to understand patient needs.
  • Assist with referrals and appointment scheduling.
  • Support care coordination activities in collaboration with providers.

Requirements

  • Active LPN/LVN license with multi-state or compact licensure.
  • At least 3 years of direct patient-facing work experience.
  • Technical aptitude with Microsoft Office Suite.
  • Excellent written and verbal communication skills.
  • Must have active high-speed Wi-Fi and a home office or HIPAA-compliant workspace.
  • Sedentary role requiring prolonged sitting, clear speaking/listening by phone, and real-time note-taking.
  • Epic experience is preferred.
  • Bilingual ability is preferred.
  • Additional single-state LPN licensures are preferred.
