Description

  • Receive and coordinate hospice referrals from physicians, hospitals, facilities, and community sources.
  • Lead pre-admission referral management and coordinate client admissions with branch operations and sales teams.
  • Verify insurance eligibility, benefits, authorizations, and required billing documentation.
  • Collect and enter accurate clinical, referral, insurance, and admission information in HCHB.
  • Explain hospice services, benefits, philosophy, rights, and financial details to clients and families.
  • Schedule hospice informational meetings and coordinate equipment, comfort kits, and transitions of care.
  • Monitor referral calls, fax lines, and portals; follow up with pending referrals and non-admissions.
  • Maintain accurate, complete, and confidential client records in compliance with HIPAA and applicable regulations.
  • Build relationships with referral sources and collaborate with clinical, sales, billing, and non-clinical teams.
  • Attend continuing education and remain current on hospice procedures, regulations, and referral requirements.

Requirements

  • Four-year college degree.
  • At least 2 years of hospice experience and/or training.
  • HIPAA-compliant home office for remote work.
  • LPN certification preferred but not required.
  • Medical office, customer service, intake, or inside sales experience preferred.
  • Strong verbal and written English communication, organization, interpersonal, negotiation, and customer service skills.
  • Ability to manage multiple priorities and work effectively in a demanding, high-stress environment.
  • Ability to collaborate with clinical, sales, operations, referral, physician, hospital, and facility personnel.
  • Knowledge of hospice benefits, services, philosophy, Medicare/Medicaid requirements, and regulatory standards.
  • Proficiency with Microsoft Office Suite, including Excel and Teams; willingness to learn HCHB.
  • Ability to work the specified evening and weekend schedule and at least one major and two minor holidays annually.
  • Ability to sit, stand, bend, lift, and move, and to work with ill, disabled, and dying clients.
