Description

  • Implement products and execute ACO initiatives under guidance from Market Leadership.
  • Support wellness visits, transitions of care, and active management efforts within partner practices.
  • Help practices meet goals outlined in jointly developed Practice Transformation Plans.
  • Train office staff, physicians, and providers on processes and technologies in coordination with Aledade's value-based care curriculum.
  • Continuously assess the training needs of practices and staff.
  • Build and maintain relationships with physicians, providers, and healthcare system executives.
  • Support financial and quality performance across practice metrics.
  • Listen to practice concerns and relay key feedback to managers and leads to identify improvement opportunities.

Requirements

  • Must be a resident of New Jersey.
  • Graduate of an accredited college or university, or have 4-6 years of relevant medical practice management experience in lieu of education.
  • 3 years of experience using Electronic Health Records (EHR) for clinical or practice management processes.
  • Excellent oral and written communication skills, including public presentation skills.
  • Strong organizational, project management, and team-building skills.
  • Familiarity with process mapping and workflow analysis tools.
  • Excellent computer skills and willingness to learn additional software applications.
  • Ability to travel to practices up to 25% within the market, including about one onsite visit per week.
  • Significant administrative experience in a clinic setting is preferred when substituting for education.
