Description

  • Collect and analyze patient data, including readmission statistics, to identify trends, patterns, and risk factors.
  • Categorize and record common themes from patient care records, helpline interactions, and clinical outcomes.
  • Develop and propose data-driven alternatives to existing care protocols to prevent readmissions.
  • Recommend adjustments and interventions to care teams based on data analysis and patient trends.
  • Collaborate with nursing, therapy, social work, and other healthcare professionals to gather care insights.
  • Provide feedback to clinical staff to help refine patient care strategies.
  • Work with clinical and educational leaders to align training programs with current data findings.
  • Suggest improvements to training materials based on observed gaps in care protocols.
  • Monitor and evaluate the effectiveness of readmission prevention initiatives and identify areas for improvement.
  • Prepare and submit reports on readmission rates, preventive measures, and care improvements to senior management and external partners within required timelines.

Requirements

  • Bachelor’s degree in Nursing; Master’s degree preferred.
  • Valid and current California RN license.
  • Minimum of 3 years of clinical experience.
  • At least 1 year of experience in home health or a similar setting.
  • Experience analyzing patient care trends and making data-driven care recommendations.
  • Strong analytical and problem-solving skills with the ability to interpret complex datasets.
  • Excellent communication and collaboration skills for multidisciplinary teamwork.
  • Commitment to improving patient outcomes through proactive data analysis and protocol improvements.
  • Must reside within California.
  • Ability to comply with federal, state, and local home healthcare and patient privacy regulations.
