Description

  • Complete health screenings and intake appointments with patients by phone and sometimes in person using a standard set of questions.
  • Build rapport quickly so patients feel comfortable sharing accurate health information during 15–20 minute calls.
  • Schedule intake appointments directly in Healthie and follow up to confirm appointments and maintain show rates.
  • Ask open-ended questions to understand patient needs and connect them to the appropriate next step, such as a doctor visit or Community Health Worker.
  • Enter patient information into the records system in real time during calls and complete all documentation before the end of each shift.
  • Follow HIPAA privacy requirements and obtain formal patient consent for every call.
  • Manage a daily schedule, space appointments appropriately, and meet target show rates and other performance goals.

Requirements

  • High school diploma or equivalent required.
  • A degree in Human Services, Public Health, or Behavioral Health is a plus.
  • Fluency in English and Spanish, or another language, is strongly preferred.
  • Experience in healthcare, social services, care platforms like Healthie, or community outreach is helpful but not required.
  • Comfort with speaking to a wide range of patients by phone and working with web- and app-based systems.
  • Ability to work independently, stay organized, and meet goals while working remotely with limited supervision.
  • Willingness to complete full training, including call practice, documentation training, and role-playing, with an 80% or higher score on the final assessment.
  • Must pass a drug test and background check before starting.
  • Ability to sit, stand, and walk regularly, use a computer while staying focused on conversations, and work from a quiet space with reliable internet.
