Description

  • Execute person-centered care plans for members with serious, high-risk illnesses and adjust plans as needs change.
  • Serve as a bridge between patients and billing practitioners by coordinating care, provider availability, rescheduling, and post-discharge support.
  • Conduct assessments to identify social, cultural, and health-related barriers and create action plans to address unmet needs.
  • Help members and caregivers navigate the Nashville healthcare system, including transportation, telehealth access, and self-advocacy with specialists.
  • Support patient communication templates, workflow automation, technical troubleshooting, and accurate documentation in the case management system.
  • Refer members to community-based resources and maintain partnerships with local Nashville organizations to connect them to services.
  • Use motivational interviewing and education to support treatment adherence, emotional well-being, and caregiver/family support.
  • Maintain detailed logs of member interactions, time spent, and activities performed to meet Medicare documentation standards.
  • Drive to member homes and public locations within a 50-mile radius of Nashville for in-person visits and support.
  • Work independently in a remote team environment while managing multiple secure systems across computer, tablet, and smartphone.

Requirements

  • Community Health Worker (CHW) certification preferred; CHW background or completed CHW training strongly encouraged.
  • 3+ years of relevant community outreach, facilitation, or healthcare experience.
  • Experience navigating high-risk chronic conditions or complex care management is a major plus.
  • Willingness to complete and maintain CMS-mandated training and state-applicable requirements.
  • Strong computer skills with the ability to use web-based and app-based systems with ease.
  • Deep understanding of socio-economic and public health challenges affecting disenfranchised populations in Nashville.
  • Excellent written and verbal communication skills, with an outgoing personality and ability to motivate and influence others.
  • High school diploma or GED required; college degree preferred.
  • Valid driver’s license, reliable transportation, and a flexible schedule for occasional work outside regular business hours.
  • Commitment to a drug-free workplace and willingness to complete pre-employment substance abuse testing and background checks.
