Description

  • Provide telephonic, video, and SMS-based outreach, engagement, and education to vulnerable Medicaid patients in California.
  • Work with a multidisciplinary care team, including Community Health Workers, Care Coordinators, and Behavioral Health Therapists, to support primary care practices.
  • Support patients and providers during transitions of care.
  • Deliver clinical services that support day-to-day operations and safe program practices.
  • Participate in quality improvement activities to improve quality, cost-effectiveness, and return on investment.
  • Use analytics dashboards and registry data to proactively identify and outreach patients with medication rejections, admissions/readmissions, adherence needs, or high risk for adverse events.
  • Lead, supervise, and mentor pharmacists and pharmacy technicians.
  • Help patients obtain medications and durable medical equipment after initial rejection.
  • Support medication adherence and chronic disease education for patients.
  • Address patient barriers to medication access and identify patients who can benefit from Waymark’s assistance.

Requirements

  • PharmD with an active advanced practice license in good standing in California.
  • Completion of an accredited PGY1 residency in a related discipline.
  • 5+ years of pharmacy practice experience, including direct patient care interventions in an ambulatory setting among vulnerable populations.
  • Experience in multidisciplinary care management.
  • Positive referrals from prior colleagues.
  • Completion of PGY2 residency training in ambulatory care, geriatrics, pharmacotherapy, or internal medicine is preferred.
  • Strong analytical skills and ability to navigate dashboards and registry data for proactive outreach campaigns, including HEDIS gaps in care.
  • Experience with medication management through collaborative practice agreements with primary care providers is preferred.
  • BPS board certification is preferred, especially BCPS and/or BCACP.
  • Experience serving vulnerable populations with skills such as motivational interviewing, harm reduction, and behavioral health is preferred.
  • Leadership experience supporting pharmacists, pharmacy technicians, and/or other care team members is preferred.

  • Salary range varies by location, from $96,000 to $164,000.
