Description

  • Provide direct supervision of the daily activities of outreach and care coordinators in the market.
  • Participate in patient outreach, care coordination, and provider communication alongside the team.
  • Coach, mentor, and support coordinators to ensure competency and compliance with workflows.
  • Monitor performance dashboards and use data to assess progress toward team and individual goals.
  • Partner with Operations and Learning & Development to hire, train, deploy, and support local coordinators.
  • Foster primary care provider partnerships through communication and in-person or virtual meetings.
  • Build and maintain community partnerships through meetings and community events.
  • Maintain knowledge of community resources and programs that can improve members’ care.
  • Collaborate with multidisciplinary care team members to execute cross-team workflows and projects.
  • Contribute to an inclusive, positive local team culture and represent Waymark’s values.

Requirements

  • Deep commitment to improving healthcare for underserved populations.
  • Experience supervising care coordinators, patient navigators, customer service, outreach specialists, or similar roles.
  • Strong understanding of Medicaid and MCOs, including barriers to healthcare access and quality for diverse underserved populations.
  • Proven ability to monitor and optimize metrics using Excel or other data tools to drive continuous improvement.
  • Demonstrated ability to collaborate and communicate effectively in a multidisciplinary team setting.
  • Experience with healthcare data and/or electronic health records.
  • Ability to build and supervise a team of care providers while fostering professionalism and a positive culture.
  • Ability to travel locally across Chicago and within the market, with up to 30% in-person travel required.
  • Bachelor’s degree in health administration, health management, public health, business administration, or a related field preferred.
  • Longtime resident of the Greater Chicago area with knowledge of MCOs and community resources preferred.
  • Multilingual skills are a plus.
  • COVID vaccination required, with reasonable accommodations considered for disability or sincerely held religious belief.
  • Employment is contingent on successful completion of a background check, including employment history and degree verification when applicable.
