Description

  • Coordinate and support a nationwide network of Residential Care Communities, respite care partners, and franchise locations.
  • Assist with sourcing, evaluating, and onboarding new partners by collecting documentation, credentialing materials, and compliance agreements.
  • Maintain accurate partner records, service areas, and operational updates in internal systems and the CMS Portal.
  • Track contract renewals, amendments, and expirations to maintain uninterrupted service coverage.
  • Serve as the primary daily contact for partner inquiries and routine support needs.
  • Manage referral workflows and escalate urgent or complex cases as needed.
  • Conduct routine partner outreach and performance check-ins to monitor service standards and engagement.
  • Collaborate with Care Navigation, Finance, and Technical teams to resolve workflow, billing, and payment issues.
  • Help draft and maintain SOPs, workflow guides, and escalation pathways.
  • Track operational metrics and prepare performance reports and dashboards for leadership review.

Requirements

  • 2–3+ years of experience in operations, healthcare operations, care coordination, vendor support, or network administration.
  • Experience working with external partners, vendors, or providers; healthcare or social support setting preferred.
  • Strong attention to detail and the ability to manage multiple operational tasks simultaneously.
  • Comfort working with spreadsheets such as Excel or Google Sheets, basic dashboards, and system portals.
  • Strong written and verbal communication skills.
  • Ability to work cross-functionally and manage daily tasks in a remote environment.
  • Experience or familiarity with respite care, residential care, skilled nursing care, home health, or caregiving services preferred.
  • Basic familiarity with contract coordination, administrative compliance, or credentialing processes preferred.
  • Ability to work fully remote, with occasional travel expected for team meetings or collaboration events.
  • Work authorization without visa sponsorship, as the company is unable to provide sponsorship.
