Description

  • Curate relative provider lists for members who need readily available care.
  • Outreach to providers and facilities to identify relevant, personalized care options that meet member requests, SLA requirements, and access standards.
  • Collaborate with RNs on complex cases to help route care effectively.
  • Discover and communicate trends to leadership to improve processes and the member experience.
  • Use available resources to navigate complex cases.
  • Confirm and update provider data.
  • Document tickets accurately and thoroughly.
  • Stay current on updates, processes, and workflows.
  • Comply with all applicable laws and regulations.
  • Perform other duties as assigned.

Requirements

  • 1+ years of experience with care coordination and navigation in a healthcare setting.
  • 1+ years of experience using a Chromebook and Google Workspace.
  • Must reside in Arizona, Florida, Georgia, or Texas.
  • Experience working in a collaborative environment (preferred).
  • Ability to work remotely.
  • Pay transparency: set pay rate of $19.00 per hour.
  • Monthly vacation accrual at a rate of 15 days per year.
