Description

  • Independently manage a caseload of complex, multi-need clients through collaborative long-term case management.
  • Support client engagement as they move through different levels of care using Motivational Interviewing and related strategies.
  • Manage client crises by completing brief risk and needs assessments, safety planning, and de-escalation.
  • Respond to incoming crisis calls with brief, evidence-based coping support.
  • Communicate professionally with clients, providers, health plans, and navigators by email, phone, and occasional text.
  • Use matching software, facility databases, health plan directories, and internet searches to identify appropriate treatment options.
  • Administer and monitor client symptom measures such as PHQ-9 and GAD-7.
  • Develop personalized care plans and present cases in Multidisciplinary Team Meetings.
  • Coordinate care with preferred facility network providers before, during, and after treatment, including discharge planning.
  • Participate in on-call rotation and complete screenings for Lyra’s DBT program.
  • Provide feedback to leadership to improve client experience, operational efficiency, and outcomes.

Requirements

  • Master’s degree in clinical social work or equivalent.
  • Active independent clinical licensure required, such as LCSW, LMFT, LPC, or LPCC.
  • Candidates without a state license to independently deliver mental health services will not be considered.
  • Unable to provide supervised hours toward licensure.
  • Significant experience managing high-risk clinical situations, including suicidality and mandated reporting.
  • Experience navigating health plans and encouraging engagement with clients who have escalating mental health needs.
  • Proficiency in case management and delivering specialized care for clients with diverse needs.
  • Knowledge of higher levels of care and community mental health resources.
  • Familiarity with health plan procedures and medical necessity criteria.
  • Ability to work independently in a remote environment and collaborate effectively within a small clinical team.
  • Must be a full-time resident of the United States and able to work only from within the U.S.
  • Must be bilingual and Spanish-speaking.
  • Ability to work Monday through Friday, 9:00am to 5:30pm PT.
