Description

  • Provide telephone triage support for hospice patients and families after hours.
  • Assess patient conditions and determine the appropriate next steps in care.
  • Coordinate with on-call teams to support care delivery and resource needs.
  • Document calls, interventions, and communications accurately in the EMR.
  • Maintain a calm, professional, and empathetic demeanor during urgent calls.
  • Support timely patient care coordination during non-core hours.
  • Work independently while collaborating with the broader care team.
  • Handle a high volume of calls in a fast-paced remote environment.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience is strongly preferred.
  • Comfort using technology and electronic medical records (EMR) for documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to handle stress and multitask while receiving calls.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts.
  • Capacity to manage a call volume of at least 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

  • Base pay of $27 per hour, with opportunities to increase to up to $30 per hour within the first 6 months.
  • $2 shift differential for overnight shifts and $2 shift differential for weekend shifts.
