Description

  • Provide telephone triage support for hospice patients and their families after hours.
  • Assess patient conditions and determine appropriate next steps.
  • Coordinate care and resources with on-call teams and client care teams.
  • Document calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, compassionate demeanor during urgent calls.
  • Support timely patient care coordination and timely interventions during non-core hours.
  • Work independently in a remote environment while maintaining consistent attendance and communication.
  • Manage a fast-paced call volume while balancing documentation and multitasking.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience strongly preferred.
  • Comfortable using technology and electronic medical records (EMR) for documentation.
  • Able to type documentation and notes accurately in a fast-paced environment.
  • Able to handle stress and multitask during high call volumes, including at least 5 calls per hour on weekdays and up to 8 per hour on weekends.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote setting.
  • Reliable internet connectivity and consistent attendance during all scheduled shifts.
  • Comfortable with remote work requirements and active communication throughout shifts.

Benefits

  • Base pay of $25 per hour after training, with potential to earn up to $28 per hour within the first 6 months.
  • $1 shift differential for overnight shifts.
  • $1 shift differential for weekend shifts.
  • 3 weeks of paid remote training.
