Description

  • Provide telephone triage support for hospice patients and their families after hours.
  • Assess patient conditions and determine the appropriate next steps for care.
  • Coordinate with on-call teams to support timely interventions and resource planning.
  • Document calls, communications, and interventions accurately in the EMR.
  • Maintain a calm, professional, and empathetic demeanor during urgent situations.
  • Support patient-centered care by following client-customized protocols.
  • Work independently while staying in active communication with the care team.
  • Help ensure continuity of care during non-core hours for hospice services.

Requirements

  • Active multistate Registered Nurse (RN) license.
  • Hospice, palliative, or end-of-life care experience strongly preferred.
  • Comfort with technology and electronic medical records (EMR) for call documentation.
  • Ability to type documentation and notes in a fast-paced environment.
  • Ability to manage stress and multitask while handling a high volume of calls.
  • Strong communication and critical thinking skills.
  • Ability to work independently in a remote environment.
  • Consistent attendance, active communication, and reliable internet connectivity required for all scheduled shifts.
  • Comfortable handling a minimum of 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

  • Base pay of $25 per hour after training, with potential to earn up to $28 per hour within the first 6 months.
  • $2 shift differential for overnight shifts.
